Building a Healthier Future for All
HealthVisions Midwest has been committed to advancing health and wellness in underserved communities for over 27 years. With a strong presence in Indiana, its offices in Fort Wayne and Hammond serve as hubs for outreach, advocacy, and impact.
Through hands-on health education, culturally responsive programs, and strong community partnerships, HealthVisions Midwest addresses chronic disease, access to care, and wellness disparities. Certified Community Health Workers lead classes in both English and Spanish to help participants manage diabetes, heart disease, reduce stress, and make informed lifestyle choices that support long-term health.
In addition to education, the team offers interpretation services for Spanish-speaking residents, healthy food and vouchers, connections to essential resources, and free transportation in Lake County for seniors and individuals with disabilities. HealthVisions Midwest also facilitates local health coalitions that bring community partners together to address barriers and support sustainable change.
The mission remains clear: to expand access, strengthen communities, and create a future where everyone has the opportunity to live a healthier life.
